Service Description
Auto Accident Massage is provided to support recovery from motor vehicle collision–related injuries when massage therapy is considered medically necessary. Treatment is billed to insurance at four (4) therapeutic units ($50 per unit) using CPT code 97124. If clinically indicated, heat or cold therapy may be applied and billed separately under CPT code 97010. Medical necessity is based on symptoms such as pain, reduced range of motion, muscle tension, or limitations in activities of daily living (ADLs). The initial visit includes a clinical assessment to document these factors and establish an appropriate treatment plan. Assessment time may be billed separately when applicable. Reassessments are typically performed on the 5th visit and then every 10 sessions or once per month, whichever comes first. A referral or prescription from a physician, chiropractor, naturopath, or acupuncturist is preferred to support medical necessity, but Oregon law allows patients to self-refer for massage therapy after an auto accident. No payment is required at the time of booking. A credit or debit card is required to reserve the appointment, as insurance does not cover missed sessions. No-shows or late cancellations will be charged at the full $150 cash rate. Additional charges for clinically indicated modalities (such as heat or cold therapy) may be added and billed to your insurance. All billing and claims are handled directly by Earth Breath Yoga.
Cancellation Policy
By scheduling an Auto Accident Massage, patient agrees to the following. Service listed as 60 minutes at $200 & billed to insurance at 4 units ($50/unit) under CPT 97124. Heat/cold therapy (CPT 97010) may be billed separately when clinically indicated. Assessment time during initial visit or reassessments may be billed separately when appropriate. Required Documentation: Auto Accident Intake Form, accident date, claim number, insurance company & adjuster contact must be provided before or at first appointment. If documentation not provided, card on file will be charged $150 cash rate & insurance not billed. Medical Necessity: Massage billed to insurance only when medically necessary & related to accident injuries such as pain, reduced range of motion, muscle tension, or limitations in ADLs. If symptoms resolve or no longer meet necessity, insurance coverage may end. Referrals: Referral or prescription from MD, DC, ND, or LAc preferred to support necessity but not required under Oregon law. Card on File: No payment due at booking; credit/debit card required to reserve. Insurance does not cover missed or late-cancelled appointments. Cancellations & No-Shows: 24 hours’ notice required. If missed or cancelled with less than 24 hours’ notice, $150 cash rate charged to card. Insurance not billed for missed sessions. Denied, Exhausted, or Non-Covered Claims: If insurance denies, exhausts benefits, or refuses payment, patient responsible for full session amount. $25 Administrative Processing Fee charged per denied or non-covered claim to cover documentation, SOAP notes & submission costs. Reassessments: Performed at first visit, around 5th session, & then every 10 sessions or approx. once/month. Time may be billed separately. Modalities: Heat or cold therapy provided when clinically indicated & billed separately under appropriate CPT codes. Agreement: By booking, patient accepts responsibility for missed appointments, lack of paperwork, denied or non-covered claims & related administrative fees. Earth Breath Yoga may discontinue treatment if policies not followed or medical necessity no longer supported.
